The Health and Human Services Program assists individuals and families to stabilize their lives and work toward self-sufficiency. It provides community health, case management, and emergency services. The Program is available to low- and moderate-income residents of Rogers Park. It has bilingual English/Spanish-speaking staff. The program offers the following services to qualified individuals:

Program Highlights:
- Every month, more than 80 individuals receive affordable dental services at our on-site clinic. About 20% of all visits are reserved for patients with HIV/AIDS.
- Each year, more than 600 individuals receive case management to stabilize their lives. They receive help to secure health care, housing and other resources.
- About 130 women and children who are survivors of domestic violence participate in support groups and counseling each year.
